If the country of origin or provenance does not provide for the issuance of an equivalent to the above document, this may be replaced by an affidavit or, in the case of Member States where this does not exist, a solemn declaration submitted by the person concerned before the competent court or administrative authority or, as the case may be, before a notary of the country of origin and provenance, who shall issue a certificate of performance of the affidavit or the solemn declaration. Documents issued in accordance with the above may not be submitted later than three months from the date of issue. If issued by a local authority, it is sought ex officio.
