2Certificate of Neurologist-psychologist’s or pathologist’s proving the state of mental health in case of a person, or representatives and manager in case of a limited company (SA) and a limited liability company (Ltd.) and of the partners in case of a general partnership and a limited partnership (EU) or the members of the board of directors in the case of associations.Health certificate

The certificate shall be delivered by the doctor, to the person who requested examination in a sealed envelope and shall be valid as long as the licence issued, and shall be lodged only in the event of the expiry of three years from the former; this supporting document shall not be required when the requested authorisation is to be issued within three (3) years of the authorisation, for which all the supporting documents have been submitted.

3A Solemn Declaration according to Law 1599/86 of the interested parties stating that the declaration does not fall under the prohibitions laid down in Article 18 of Law 2168/1993 and that the declarant is not a fugitive or an absconder, in the case of an individual company, or representatives and managers, in case of a limited company (SA) and a limited liability company (Ltd.) and the partners in the case of a general partnership and a limited partnership.Solemn Declaration
