Application – Solemn declaration of the applicant (article 8 of Law 1599/1986) (Request description: Declaration of temporary and occasional provision of services of the occupational optician - optometrist profession)

In the event that neither the profession nor the corresponding education is regulated by law in the Member State of legal establishment: the person concerned must have acquired professional experience of at least one year in the last ten years preceding the provision of services in Greece
