Conditions

Submission of Application – Solemn Declaration of the interested party (natural or legal person) on the subject: "Statement of temporary/cross-border provision of slimming units - dietary units services".

The interested party must have the required infrastructure.

If no authorization is provided for the specific activity by the Member State of establishment, a certificate from the competent authority of the Member State of the establishment regarding the legal incorporation of the company and its legal representative shall be provided.Certificate
